At Fall Creek Pain Management, we’re committed to helping you find the pain care you need, and we do accept self-referrals. One of our team members will promptly contact you to schedule a free, no-obligation consultation with one of our providers. These consultations can typically be scheduled within one week, depending on your availability. 

Initial Consultation

Following your consultation, if you feel that our clinic is a good fit for your pain management needs and our providers determine that our care model is appropriate for you, you will be asked to sign a medical records release form so we may obtain your prior records. Once those records are received and reviewed, your initial appointment can generally be scheduled within 2–3 weeks, depending on the timing of record receipt.
